Designing a Simple Survey Form
For each new campaign, create a dedicated survey form. Begin by selecting the appropriate form type during the campaign creation process, and then enter the necessary details for your survey questions.
NOTE
You can also duplicate and rename an existing survey for your new campaign. Find the form on the Review Builder Forms tab and select the Copy Review Form action from the Actions section. You'll need to give the form a new name to associate with your new campaign.
- Login to your Chatmeter account in LocationHQ.
- Go to Reputation > Review Builder and select the Forms tab.
- Select +Add New Survey to display the different form types.

When you select a form type, the resulting page provides customizable options for the form in the left panel and a preview of the form in the right panel. - Simple Form—Use a basic survey form to ask questions. Fields include a question, a star rating selection field, a location selector, and a customer text input field.
- Net Promoter Form—Use a specific survey to ask a Net Promoter Score (NPS) style question. Fields include a heading, an NPS question, a location selector, and a customer text input field.
- Fill out the left panel with the information you want in your survey. The only required information is Title, which will be listed on the Forms tab as the Survey Name.

You can also select Visibility for your survey. If you select Shared, anyone on your account can use the campaign content. Only you and the account Admins have access if you select Private.
As you fill in the information, it populates in the scrollable preview on the right, showing the survey and giving access to the thank you page preview. You can edit the style and format of the Simple Form and the Net Promoter Form and thank you pages, which requires knowledge of HTML. - Heading

- When you're satisfied with the results, select Save to return to the Review Builder Forms tab, where the new survey form is now listed.
