If you have Surveys enabled on your account, you can use Survey Creator to design complex surveys. You'll need to create a new survey form for each campaign. When you create a campaign, select the survey form to use with your campaign and supply survey details.

You'll initiate the form, select a form type, and supply the details for your survey questions.
- Login to your Chatmeter account in LocationHQ.
- Go to Engagement to select the Surveys tab.
- Select +Add New Survey to to see the different survey types you can select.
The Add New Survey page has seven options for surveys. Each template has predefined questions that can be used as-is, or you can customize a survey to fit your needs better. - Custom Survey—Lets you design a customized survey from scratch. Follow the steps in Creating a custom survey from scratch.
- NPS—Helps you measure customer loyalty and satisfaction. It captures the likelihood that a customer would recommend your product or service to others.
- CSAT—Gauges customer satisfaction using a 5-point scale to determine business health.
- Employee Engagement—Focuses on gathering insights on your workforce's satisfaction, motivation, and overall engagement.
- Product Satisfaction—Focuses on gathering feedback regarding the customer's experience with buying a product or the quality of the product itself.
- Review Builder
- Simple Form—Create a basic survey form to ask questions using Review Builder (Legacy). Fields include a question, a star rating selection field, a location selector, and a customer text input field.
- Net Promoter Form—Create a simple survey form to ask a Net Promoter Score (NPS)-style question using Review Builder (Legacy). Fields include a heading, an NPS question, a location selector, and a customer text input field.
TIP
You can copy an existing survey for a new campaign. Find the survey on the Surveys page and select the Copy Review Form (duplicate) icon. You'll need to give the survey a new name to associate with your new campaign.
Creating a custom survey from scratch
- Select the Custom Survey type. The Survey Creator Designer tab displays.
- Start by giving your survey a title and, optionally, a description. You can also add a logo to the survey using the image icon. The logo will also appear on your survey's thank you page.
The right panel is a menu of advanced settings, including General settings and customizable features for the Thank You Page. You can hide it with the Hide Panel icon if it is distracting. - To add a question, select a question type from the left panel. You can also choose the question type to drag and drop to the Add Question field or select Add Question to display the question dialog box. Select from the input type selector to define the type and method of response to your question. You can enter a heading in the Description field.
In this example, we selected the Rating Scale type. Chatmeter uses the first rating scale question to display an overall average rating for the survey. It's not over time.
Explore the question types on the left menu to become familiar with the options. The question dialog boxes include the type selector, an icon to duplicate the current question, a toggle indicating whether a response is required, and a delete option.
Question types include the following response formats for responding to a question:

Name
Respondent inserts their name. If a survey is sent using a direct link, this field creates a contact within Surveys. If the survey is sent by a contact email, this field will not be displayed to the recipient.
Email
Respondent inserts their email address. If a survey is sent using a direct link, this field creates a contact within Surveys. If the survey is sent by a contact email, this field will not be displayed to the recipient.
Location Selector
Respondent selects their location. This selector doesn't display if Chatmeter already has a location associated with this user. If a survey is sent using a direct link, this field creates a contact within Surveys.
NPS
Respondent selects a response to an NPS-style question (how likely are you to recommend…). Chatmeter uses this response to populate the Surveys NPS widget.
Radio Button Group
Respondent selects one item from among a group of items.
Rating Scale
The respondent selects a rating of 1 to 5, stars, or smileys. Chatmeter uses the first rating scale question to display an overall average rating for the survey. It's not over time.
Checkboxes
Respondent selects as many items as you offer on a list. You can include another option eliciting a text-input response.
Dropdown
The respondent selects one item from the list that appears.
Multi-Select Dropdown
Respondent selects one or more items from the list that appears.
Yes/No (Boolean)
The respondents' response is yes or no.
Image Picker
Respondents select among images you've uploaded to answer your question.
Ranking
Respondents rank their responses in preferential order.
Single-Line Input
Respondents enter text in a single-line text field. By default, the number of characters isn't limited. Still, you can configure a maximum character limit through Validation in the right menu and add an error message to display if a user exceeds the allowed character number.
Long Text
Respondents enter text in the multi-line text field. By default, the number of characters isn't limited. Still, you can configure a maximum character limit through Validation in the right menu and add an error message to display if a user exceeds the allowed character number.
Multiple Textboxes
Respondents enter text in more than one labeled field.
Panel
Respondents enter responses to more than one question of varying format types presented in one panel.
Dynamic Panel
Respondents enter responses to more than one question of varying format types presented in one panel. The next question is offered dynamically to the respondents after they complete a question.
Single-Select Matrix
The respondent selects one response from a matrix.
Multi-Select Matrix
Respondent selects one or more responses from a matrix
Dynamic Matrix
Respondents enter responses to more than one question of varying format types presented in a matrix. The next question is offered dynamically to the respondents after they complete a question.
HTML
Use this type to customize HTML content.
Expression (read-only)
Use this type to create rules based on responses to previous questions.
Image
Use this type to insert an image.
Signature
Use this type to allow the survey respondent to draw a signature - Type your question in the question1 text input field.
- Select the Preview tab to see what your survey form will look like.
You can view the survey on various device types through the Select the Device Type icon on the top right. - On the Designer tab, continue adding as many questions as you need
- Create customized content for your survey thank you page by editing the Message and the Provider Links on the Thank You Page in the right navigation menu. It's important not to skip this step because you can select listing providers for which you'd like a link included on the page. The link the respondent receives is based on the location associated with them (by a contact with a location associated with it or by the location selector). Your customers can click the link to the provider listing and add a review.
- Select the Save icon after adding content to your survey. You'll receive a success notification, and your survey appears on the Survey tab. To edit the survey after you've saved it, find it and select the Edit icon.
Creating a survey form using Review Builder (Legacy)
If you selected Review Builder (Legacy) (Simple Form or Net Promoter Form), follow these steps to provide the details for your survey.
As you fill in the information, it populates in the scrollable preview on the right, showing the survey and giving access to the thank you page preview.
TIP
Knowledge of HTML is required to edit the style and format of the Simple Form, the Net Promoter Form, and the thank you pages
- When you select a form type, the resulting page provides customizable options for the form in the left panel and a preview of the form in the right panel.
Fill out the left panel with the information you want in your survey. The only required information is Title, which will be listed on the Forms tab as the Survey Name. You can also select a color theme and customize the Heading, Rating (or Net Promoter) Question, Detail Question, and Submit Text, if desired. You can also select Visibility for your survey. If you select Shared, anyone on your account can use the campaign content. Only you and the account Admins have access if you select Private. - When you're satisfied with the results, select Save to return to the Surveys tab, where the new survey form is now listed.
